Wallace Community College Athletics

The following documents are required to be provided to the Athletic Director PRIOR to any athletic participation: 

•    Acknowledgment of Athletic Handbook 
•    Acknowledgment and Consent to be Drug Tested
•    Completed Emergency Contact and Insurance Information
•    Previous Illness/Injury Form
•    Consent to Participate Form 
•    Release of Information Form (if you would like for us to speak to a third party - parents, for instance - about your academic progress)
•    NJCAA Eligibility Affidavit (freshmen and transfers) 

For the 2026-2027 academic year, these forms will be emailed to student-athletes at their Wallace student email account no later than July 15. You may also complete them online by clicking the links above and using your OneACCS credentials to log in.
