Wallace Community College Athletics

The following documents are required to be provided to the Athletic Director PRIOR to any athletic participation: 

•    Acknowledgment of Athletic Handbook 
•    Acknowledgment and Consent to be Drug Tested Form 
•    Completed Emergency Contact and Insurance Information
•    Copy of current insurance card (if applicable)
•    NJCAA Eligibility Affidavit (freshmen and transfers) 
•    NJCAA Transfer Waiver and Tracking Form (if applicable)
•    Previous Illness/Injury Form
•    Signed Consent to Participate Form 
•    Signed Release of Information Form (if you would like for us to speak to a third party - parents, for instance - about your academic progress)

Incoming student-athletes, please complete these forms, then return to your coach's attention at 1141 Wallace Drive, Dothan, AL, 36303.
